Eval Center helps consumer achieve goals

Jacob Smith attended the Evaluation Center in the summer of 2016. He was referred for weakness in the left side of his body and impaired balance following a cerebrovascular accident (CVA), the medical term for a stroke. He also has chronic kidney disease and receives dialysis treatment three days per week.

Since completing the four-week evaluation, Smith has continued to receive services as a community consumer. Every Tuesday he attends occupational therapy sessions to improve his strength, range of motion and hand dexterity. This has resulted in significant improvements. He is now able to walk without assistance and drive independently. He performs all activities of daily living and household chores using both of his hands. Due to his progress in occupational therapy he is able to operate a computer and complete tasks with Microsoft Office Word, utilize e-mail, and navigate Excel spreadsheets.

Smith is motivated, consistent and committed to maximizing his independence and maintaining a healthy lifestyle. He credits the Evaluation Center for helping him to achieve his goals and remain physically and socially active. He continues to work with VR exploring career options.
